As part of our ongoing work to improve services to residents in mid and south Essex, we at NHS Mid and South Essex want to reach out and collaborate with people who access and experience our SMI* physical health checks.

You can help us achieve this by taking a moment to take part in our survey and have your say in how our services are designed and delivered. The survey should take 10-15 minutes to complete, but it may take a little longer if you choose to write long responses.

*Patients with the following conditions will meet criteria for eligibility for SMI health checks: Those who have a diagnosis of schizophrenia, bipolar affective disorders or other psychotic disorders. Those who are prescribed lithium. For more information visit: Annual health check for people with severe mental health conditions - NHS (www.nhs.uk)

  • Take part in our survey and become part of transforming services for yourself and other people with severe mental health* issues

We are reviewing the communication that we send to residents in mid and south Essex who have been diagnosed with a severe mental illness (SMI).

Through helping us to review how we communicate with our residents you will be helping us to be able to connect with people more effectively and ensure that they can access the right services at the right time, which will prevent them from developing life limiting conditions as well as better manage existing illnesses.

The survey should take 10-15 minutes to complete, but it may take a little longer if you choose to write long responses. The survey will take you though a series of statements, information leaflets and invite letters and text, where you will be given the opportunity to tell us what you think about the material and how they could be improved.

This survey can be completed anonymously although you can join Virtual Views and become a part of transforming services for all Essex residents. Your responses will be used by the MSE ICB for the purposes of analysis and reporting on an anonymous basis. Any data shared with us, will NOT contain any information that could identify you. Reports using this information may be available to the public and anonymous quotes may be included.

Data security - Your data is protected under General Data Protection Guidelines. Further information on our Privacy Statement can be found online at Privacy - Mid and South Essex Integrated Care System (ics.nhs.uk)
