This system is being developed to ensure that health and social care professionals from different services, have a joined-up view of information as they are delivering care to you (your direct care). For example, the Shared Care Record will include information from GP practices, hospitals, community health services, mental health services, and relevant teams within local authorities.
The Shared Care Record will be a secure system that only health and care professionals involved in your care can access. Individuals receiving care will not have direct access to it. However, understanding your perspective as an individual who may access health or social care services is vital. Your insights are critical in shaping how we implement and communicate the Shared Care Record to best serve your health and care needs.

We are committed at all times to protecting your privacy and will only use information ethically and lawfully in accordance with UK General Data Protection Regulations, the Data Protection Act 2018, the Human Rights Act 1998 and the common law duty of confidentiality. The various laws and rules about using and sharing confidential information, with which the NHS Integrated Care Board will comply, are available in the guide to confidentiality in health and social care which is published on the NHS Digital website, link below.
Your responses will remain anonymous. NHS Mid and South Essex Integrated Care Board will use what you tell us for analysis and reporting. NO information you share with us will contain any details that could identify you. Any report created using the insight you provide may be available to both NHS staff and the public and include anonymous quotes taken from your responses.
