Share your experience about the NHS 40+ health checks
We wish to understand more about your overall experience of being invited and attending an NHS 40+ health check, and how your experience of having a check impacts your health and wellbeing. This allows us to provide recommendations for the development of a programme to deliver better experiences to residents of mid and south Essex.
We encourage anyone who is eligible for a health checks whether they have attended an appointment or not to please complete this survey. The survey should take about 10 minutes to complete, but it may take a little longer if you choose to write long responses.
Anonymity
Your responses will be used by the NHS Mid and South Essex ICB, Essex County Council, Southend City Council and Thurrock Council along with the providers who carry out health checks on their behalf for the purposes of analysis and reporting on an anonymous basis. Any data shared with us, will NOT contain any information that could identify you. Reports using this anonymous information may be available to the public and anonymous quotes may be included.
