Different tiers of diabetes services in mid and south Essex

Tier 1 - GP surgery

Tier 2 - Diabetes specialist nursing team, this is delivered by three different providers

  • NELFT - deliver services in Basildon and Brentwood and Thurrock
  • Provide - deliver services in Mid Essex
  • EPUT - deliver the services in South East Essex
